| Spanish abstract | Ingresa paciente del sexo masculino de 23 años de edad, al Servicio de Urgencias del Hospital Dr. Rubén Leñero con el antecedente de haber sufrido trauma cerrado con puños aproximadamente 2 horas antes. Desde su ingreso las constantes vitales se encontraron en parámetros normales, TA 110-80 mmHg, Fc 92x’, Fr 23x’,T 35.5°C. Clínicamente no presenta huellas de contusión ni hematomas, se encuentra tranquilo, refiere que sólo fue una «simple riña», sin pérdida del estado de alerta, manifiesta dolor en el dorso, aumentando de intensidad desde el incidente hasta su valoración, no datos de irritación peritoneal, ni de choque traumático, tomamos la decisión de efectuar tomografía abdominal contrastada, donde apreciamos una gran colección retroperitoneal desplaza asas de colon y de intestino delgado. Súbitamente presenta dolor abdominal intenso por lo que decidimos realizar laparotomía exploradora. Descubrimos un gran hematoma retroperitoneal, zona II izquierdo, con un gran contenido de orina y sangre 1,000 cc aproximadamente, dependiente de una masa quística renal, la cual contiene tejido renal sangrante, se realiza excisión del quiste y se completa la nefrectomía, el paciente evoluciona sin complicaciones, egresando a los 5 días |
| English abstract | Male 23 years old arrive at emergency service of Dr. Ruben Leñero Hospital, he suffer stroke hit approximately 2 hours before. Since entrance the vital signs found stables TA 110/80, FC 92x’, T 35.5°C. Clinicaly without signs of contusion or hematoma, the patient is quiet, he say that only was a «simple dispute» with loss alert status, with pain in dorsum, increasing of intensity since the dispute at moment of to be valuated, no facts of peritoneal irritation, or traumatic shock, we take CAT scan abdominal with contrast, appear big retroperitoneal collection displace the colon and small bowel. Suddenly has intense pain and is carried at operating room and discovered a big retroperitoneal hematoma zone II left, with urine and blood 1,000 cc approximately depending of a cystic renal mass, contain tissue renal bleeding, the surgical management is excision of cyst and nefrectomy, the patient is eggressed in 5 days without complications |
