Spanish abstract | La alcalosis posthipercápnica (APH) es una complicación frecuente de la ventilación mecánica (AMV) en pacientes con EPOC exacerbado. Metodología: Estudio retrospectivo de 232 expedientes con EPOC exacerbado; se seleccionaron los 30 que requirieron AMV y se excluyeron 10 por ser AMV no invasiva. Resultados: Edad promedio: 74 ± 11 años, APACHE: promedio 18 ± 7 (8-38), 13 hombres (65%); 16 sobrevivieron, mortalidad: 20%. Se dividieron en pacientes sin PHA (n = 11, 55%) y con PHA (n = 9, 45%). Se obtuvo CO2 previo a AMV: 70 ± 27 (31-114) y 87 ± 11 (68-99) en pacientes sin PHA y con PHA respectivamente (p = 0.09), día 1: 44 ± 6 (30-50) y 61 ± 19 (35-89) con p = 0.01, día 3 y 6 sin diferencia signifi cativa. HCO3: los valores significativos fueron el día 1, 3 y 6: día 1, 26 ± 3.9 vs 37 ± 7, p = 0.0001, día 3: 27 ± 4 vs 38 ± 5.5, p = 0.0001 y día 6: 30 ± 5 vs 41 ± 6, p = 0.0001. Mortalidad: 18.2% y 22.2% con PHA y sin PHA respectivamente, sin diferencia. La estancia hospitalaria fue de16 ± 4 vs 26 ± 15 y p = 0.04. Conclusiones: Pacientes con EPOC y PHA tienen mayor estancia hospitalaria y días de AMV. Hay incremento persistente del bicarbonato, sin relación a cambios en el PaCO2, diuréticos, pérdida gastrointestinal o administración de HCO3 |
English abstract | Alkalosis post hipercapnia is a complication (APH) is a frequent complication of mechanical ventilation (MV) in exacerbated COPD. Methods: Retrospective study of 232 fi les with exacerbated COPD; 30 were selected because they used MV but 10 were exclude as it was non invasive. Results: Mean age: 74 ± 11 years, mean APACHE II score: 18 ± 7 (8-38), 13 male (65%); 16 survived, mortality: 20%. We divided them in patients without APH (n = 11, 55%) and with APH (n = 9, 45%). CO2 previous to MV: 70 ± 27 (31-114) and 87 ± 11 (68-99) in patients without APH and with APH respectively.
