Spanish abstract Menos de 10% de los linfomas linfoblásticos son de precursores B (LL-B). La afectación de la piel por LL-B es relativamente infrecuente. Objetivo: informar el estudio clinicopatológico e inmunofenotípico de cinco casos de linfomas linfoblásticos con afectación cutánea; sólo uno de ellos tuvo afección de médula ósea. Pacientes y método: clínicamente los pacientes se presentaron con afección cutánea en el cuero cabelludo y en la cara, dos de ellos con úlceras grandes. La edad promedio fue de 12.8 años con límites de 4 a 24 años; fueron dos hombres y tres mujeres. Resultados: el examen histológico mostró en todos los casos afección difusa de la piel, infiltrada por células linfoides neoplásicas pequeñas a medianas con núcleos de contorno liso a irregular, cromatina blástica y numerosas mitosis. Todos los casos fueron positivos al TdT y CD79a. Cuatro de los cinco fueron positivos al CD10. El CD20 fue débilmente positivo en cuatro de los cinco y cuatro de los cinco mostraron positvidad intensa al Pax-5. El Ki-67 fue positivo 70-90 % en todos los casos. El CD34, CD1a, CD3, CD5 y CD7 fueron uniformemente negativos en todos los casos. Conclusiones: la inmunotipificación es esencial en el diagnóstico de LL-B; con quimioterapia agresiva este linfoma puede entrar en remisión
English abstract Precursor B-cell lymphoblastic lymphoma (B-LBL) accounts for less than 10% of lymphoblastic lymphomas. Involvement of the skin of a B-LBL is relatively uncommon. Objective: To describe the clinicopathologic and immunophenotypic study of fi ve cases of B-LBL with cutaneous involvement, only one of which showed bone marrow involvement. Patients and method: Patients presented clinically with tumors located on the scalp and face, two of them with large ulcers. The medium age was 12.8 years (range 4-24); there were two males and three females. Results: Histological examination showed in all cases that the dermis was diffusely infi ltrated (with no epidermotropism) with neoplastic small to medium-sized lymphoid cells, with round or convoluted nuclei, “blastic” chromatin and high mitotic rate All cases were positive for TdT and CD79a. Four of the fi ve cases were positive for CD10. CD 20 was weakly positive in four of the fi ve cases and four of fi ve cases showed strong positive nuclear staining for Pax-5. Ki-67 was 70-90% positive in every case. CD34, CD1a, CD3, CD5 and CD7 were negative in every case. Conclusions: Immunophenotyping is essential to establish the correct diagnosis of precursor B-cell lymphoblastic lymphoma. Aggressive chemotherapy is associated with a potential cure
