Spanish abstract | Introducción: Los abscesos hepáticos amibianos son una patología frecuente en México, donde se presentan con características diferentes a las encontradas internacionalmente, por lo cual en nuestro país los tratamientos y el manejo de esta patología deben ser también distintos. Aquí se propone redefinir las indicaciones para los procedimientos intervencionistas de mínima invasión en los abscesos hepáticos amibianos. En el Hospital Metropolitano “Bernardo Sepúlveda”, San Nicolás de los Garza, Nuevo León, hay un alto índice de laparotomías, así como prolongada estancia intrahospitalaria y alta morbilidad. De allí la hipótesis de que al puncionar todos los abscesos habrá mejor respuesta al tratamiento. Material y métodos: Se analizaron 29 pacientes con absceso hepático a quienes se les aplicó tratamiento tradicional (grupo control), comparándolos con 22 pacientes en quienes se puncionó el absceso 48 horas después del ingreso hospitalario (grupo protocolo). Resultados: Ambos grupos tuvieron características similares y manejo médico. En el primero se puncionó a 13.79 % y en el segundo, a 100 %. En 44.82 % del grupo control se realizó laparotomía y en 4.5 % del grupo protocolo. El primer grupo acumuló 11.96 días de estancia intrahospitalaria y el segundo, 4.76. La sintomatología en el grupo control desapareció a los ocho días mientras que en el grupo protocolo, a los dos. Discusión: Aún no se han logrado precisar las indicaciones para puncionar un absceso hepático |
English abstract | Background: Amebic hepatic abscesses are a frequent illness in Mexico, where these are different from those of the rest of the world. The treatment that we use should also be different. We propose new indications for percutaneous drainage in this pathology. In our hospital we found a high incidence in laparotomies, morbidity and lengthy hospitalization for this illness. Methods: We studied 29 cases of hepatic abscesses treated at our hospital from January 2002 to May 2004. Patients received the conventional treatment and we compared this group with another group of 22 patients who had drainage before 48 h of hospitalization, during the period from June 2004 to August 2005. Results: Both groups showed similar characteristics. The first group was drained only in 13.79 % of cases, the second in 100 %. In the first group, 44.82 % had surgery and in the second group 4.5 % had surgery. Length of hospitalization in the first group was 11.96 days and in the second, 4.76 days. Conclusions: We still have no clear indications of when to opportunely drain a hepatic abscess |
