| Spanish abstract | El traumatismo cardíaco penetrante es una seria amenaza para la vida. Menos de la tercera parte de los pacientes que lo presentan llega con vida al hospital y, de ellos, la mitad fallece. La mayoría de los cuerpos extraños penetrantes en el corazón son metálicos, causados frecuentemente por arma de fuego y, más infrecuente, por autoagresión. La penetración accidental es poco frecuente y mucho menos frecuente lo es su penetración inadvertida. Presentamos el caso de un paciente con un traumatismo cerrado de tórax con penetración inadvertida de un cuerpo extraño (alambre de cobre) en el corazón, que permaneció en el ventrículo izquierdo durante más de 3 meses. Éste es el único caso reportado en la literatura científica donde un cuerpo extraño: a) atraviesa la pared libre del ventrículo derecho, la cavidad ventricular derecha, el septum interventricular y el aparato valvular mitral; b) ocupa prácticamente todo el diámetro anteroposterior del corazón, y c) no produce ningún tipo de hemorragia (aguda o crónica) |
| English abstract | Penetrating cardiac trauma constitutes a serious threat for life. Less than the third part of all patients who suffer it arrives at the hospital alive, and half of them die. Most of the penetrating foreign bodies in the heart are metallic, and are frequently caused by firearms and rarely by self–injury. The accidental penetration is uncommon but inadvertent penetration is extremely rare. We present the case of a patient who suffered a closed–chest trauma and did not notice the penetration of a foreign body (copper wire fragment) in the heart. It remained lodged in the left ventricle for more than 3 months. This it is the only case reported in the literature where a strange body: a) crosses the free wall of the right ventricle, the right ventricular cavity, interventricular septum, and the mitral valve apparatus; b) occupies almost all the anteroposterior diameter of the heart, and c) did not produce acute or chronic bleeding |
