Resumen en español | La atención dental debe acompañar a los pacientes que reciben terapia contra el cáncer de cabeza y cuello debido a que existen efectos secundarios relacionados con la radioterapia y un mal estado bucal, así como secuelas por la cirugía ablativa. Los principales efectos desarrollados por la radioterapia incluyen xerostomía y mucositis, seguidos por trismus, caries por radiación y osteorradionecrosis. Para disminuir el riesgo de desarrollar estos padecimientos es necesaria la visita al cirujano dentista previa al tratamiento para realizar los procedimientos bajo los requerimentos de un paciente con cáncer y hacer la toma de decisiones ante dientes cariados, restauraciones extensas, enfermedad periodontal o dientes posteriores inaccesibles para una higiene diaria. Sin embargo, puede ser desafiante llevar a cabo estas tareas en el corto periodo entre el diagnóstico del paciente y el inicio de su tratamiento. De igual manera los pacientes que han terminado el tratamiento deberán ser atendidos con consideraciones especiales por la posibilidad de desarrollar osteorradionecrosis |
Resumen en inglés | Given the effects of radiotherapy on oral health and the sequelae of ablative surgery, it is recommended that patients undergoing treatment for head and neck cancers also receive dental care in conjunction with their therapy. The two main side effects of radiotherapy are dry mouth and mucositis, followed by trismus, radiation-induced caries, and osteoradionecrosis (ORN). To reduce the risk of their developing these conditions, patients should visit the dentist prior to commencing their treatment in order to undergo procedures tailored adapted to the needs of the cancer patient and to enable the dentist to decide how to proceed in the event the patient has carious teeth or periodontal disease, requires extensive restoration work or has hard-to-reach posterior teeth that affect daily hygiene. However, carrying out such tasks in the short period between the patient’s diagnosis and the start of their treatment can prove challenging. Furthermore, special attention should be paid to patients who have completed their course of treatment given the possibility of their developing osteoradionecrosis |
