Resumen en español Introduction: The causes of myocardial infarction with nonobstructive coronary arteries are heterogeneous and include different reasons. Clinical cases of myocardial infarction without obstruction represent a difficult diagnostic task where cardiac magnetic resonance is increasingly used. Objective: This research aimed to demonstrate the evaluation protocol of myocardial infarction with nonobstructive arteries. Method: The object of this study was a 49-year-old male patient who presented at the emergency department with atypical chest pain, notably elevated troponin levels. To establish the diagnosis was performed electrocardiogram, echocardiography, stress-echocardiography, coronary angiography, and cardiac magnetic resonance.
