| Spanish abstract | El modelo integral, determinado por las necesidades de salud de la sociedad, se abre paso en la educación y práctica médica frente al modelo biomédico dominante. Con la finalidad de comprender, interpretar, construir, reconstruir y presentar los significados que los actores sociales confirieron a la concepción del modelo integral de salud para la formación del médico internista de la UCLA, se realizó un estudio cualitativo basado en revisión de bibliografías, observación y entrevistas en profundidad a 5 actores sociales, utilizando el método comparativo continuo. La validación se efectuó por triangulación y el uso de múltiples fuentes. Se obtuvo diez categorías, cinco correspondieron a la dimensión “Es”: modelo biomédico, integración de los actores sociales, orientación pedagógica, recursos, concepción situacional de la Medicina Interna y cinco a la dimensión “Deber Ser”: desarrollo profesional integral, profesional humanista, líder, pertinencia social, enfoque multiinter- transdisciplinario. Se evidenció que este postgrado se fundamenta en el modelo biomédico, de tipo hospitalario, con actividades netamente curativas, interpretando el proceso saludenfermedad en la dimensión biológica, sin trascender a las determinaciones socioeconómicas, culturales y psicológicas, con orientación pedagógica centrada en el diagnóstico y tratamiento, sin coherencia con todas las políticas, planes y programas del sistema de atención de salud del país y sin interrelación con la comunidad. Se hace necesaria la reorientación del postgrado al desafío de ser pertinente, inserción en los escenarios laborales reales y a la formación de médicos internistas capaces de prestar cuidados de salud integral en los tres niveles de atención de salud |
| English abstract | Integral model, determined by the necessities of health of the community, takes place in the education and medical practices versus to the currently prevailing biomedical model. The study aims to understand, interpret, construct, reconstruct and to show the meaning that the social actors gave to the conception of the integral model of health for internal medicine specialists at UCLA. The study is qualitative in nature and takes into account a review of bibliographies, observation and interviews in-depth to 5 social actors, constant comparative method was used. Validation was made by triangulation and the use of multiples sources. Ten categories were found, five of them related to “Is” dimension: biomedical model, integration of social actors, pedagogic guidance, resources, and situational conception of internal medicine. Five were related to “Must” dimension: integral professional development, professional humanist, leader, social relevance, and multiinter- transdiciplinary approach. The study yielded a model which emphasized biomedical concept, hospital situation, curative activities, and interpreting the health-disease process in the biologic dimension. There was no reference to health as related to socio-economic factors and cultural and psychological determinations. Nor was the pedagogic orientation rise above diagnosis and treatment, without coherence with all the politics, plans and programs of the health attention system of country and without interrelation with the community. The study revealed the necessity for reorientation the training of internal medicine specialists to the challenge of being pertinent, as were as insertion in real scenarios and able to attend care integral health in three levels of attention |
