| Spanish abstract | El linfoma es una neoplasia maligna caracterizada por la proliferación de células nativas al sistema linfático, comprende de 1% al 9% de las neoplasias testiculares. La presentación de tumor testicular bilateral fue reportada inicialmente por Aberhouse en 1955. La presentación extraganglionar de linfoma Natural Killer (NK) tipo nasal, es la variante más frecuente de tumores no Hodgkin. Éstos presentan extensa destrucción vascular y necrosis, siendo el sitio más común extraganglionar la vía aérea superior, se caracteriza por mal pronóstico y diseminación sistémica con mala respuesta a los tratamientos actuales. Se presenta el caso de un masculino de 45 años de edad, con aumento de volumen testicular bilateral de 6 meses de evolución, multitratado. Presenta ultrasonido testicular y tomografía abdominal con aumento de volumen testicular, metástasis retroperitoneal y al tejido celular subcutáneo. Tiene marcadores tumorales negativos. Se realizó orquiectomía radical derecha y biopsia de testículo izquierdo. El diagnóstico histopatológico deinitivo reportó linfoma no Hodgkin de células T/NK extranasal. Se reirió al Servicio de Hematología, iniciándose tratamiento quimioterapéutico y hormonal sustitutivo. Este tumor representa 1% de todos los linfomas no Hodgkin y 5% de las masas testiculares. La edad media de presentación es de 50 años, tiene mayor incidencia en Asia y América del Sur. En el 20% de casos afecta sitios fuera de la cavidad nasal. No existe clasiicación para estadiicar. La sobrevida a 5 años es del 60% y 17% para enfermedad diseminada. No existe estándar de oro para tratamiento (orquiectomía, quimioterapia y/o quimio-radioterapia) |
| English abstract | Lymphoma is a malignant neoplasia characterized by the proliferation of cells native to the lymphatic system. It makes up 1% to 9% of testicular neoplasia. Bilateral testicular tumor presentation was irst described by Aberhouse in 1955. Extranodal nasal-type natural killer (NK) lymphoma is the most frequent variant of non-Hodgkin’s tumors and presents with extensive vascular destruction and necrosis; the most common extranodal site is the upper respiratory system. It is characterized by poor outcome and systemic dissemination and does not respond well to current treatments. A 45-year-old man presented with bilateral increased testicular volume of 6-month progression, having received multiple treatments. Testicular ultrasound and abdominal computed tomography scan revealed increased testicular volume, retroperitoneal metastasis, and metastasis to the subcutaneous cell tissue. Tumor markers were negative. Right radical orchiectomy and biopsy of the left testis were performed. The deinitive histopathologic diagnosis was extranasal- type NK/T-cell non-Hodgkin’s lymphoma. The patient was referred to the Hematology Service and chemotherapy and hormone replacement treatment were begun. This tumor represents 1% of all non-Hodgkin’s lymphomas and 5% of testicular masses. Mean presentation age is 50 years and there is a higher incidence in Asia and South America. Twenty percent of the cases affect areas outside of the nasal cavity. There is no staging classiication for this disease. Five-year survival is 60% and 17% for disseminated disease. None of the current treatments (orchiectomy, chemotherapy and/or chemo-radiotherapy) are considered the criterion standard |
