| Spanish abstract | Objetivo: La cirugía reconstructiva uretral es el procedimiento quirúrgico que permite obtener mejores resultados, sin embargo no está libre de complicaciones. Revisamos las complicaciones ocurridas en un grupo de pacientes, cómo tratarlas y cómo prevenirlas. Material y métodos: Se incluyeron 182 pacientes con diagnóstico de estenosis uretral, quienes fueron intervenidos quirúrgicamente. Realizamos 95 uretroplastías término-terminal en uretra bulbar; 40 uretroplastías con injerto empleando mucosa bucal, de las cuales 30 fueron en uretra bulbar, en posición ventral y dorsal, y 10 en uretra peneana; y 25 uretroplastías peneanas con colgajo fasciocutáneo. En 13 pacientes fue necesario realizar una perineostomía. Hubo 9 casos de rotura traumática de uretra, que fueron sometidos a anastomosis término-terminal posterior. Se realizó cirugía en 2 tiempos; en 6 casos por liquen escleroso. El seguimiento medio fue de 48 meses. Resultados: Veinte tres pacientes presentaron algún tipo de disfunción sexual. En 7 pacientes se detectó un divertículo uretral, 6 de ellos fueron tratados con una nueva uretroplastía. En 6 ocasiones apareció una fístula postoperatoria, que fue resuelta con tratamiento conservador en 4 casos y con cirugía en 2. Veinte pacientes presentaron complicaciones que precisaron tratamiento médico: 5 infecciones de herida quirúrgica, 2 infecciones urinarias, 4 orquiepididimitis, un embolismo pulmonar y una neuropraxia del nervio peroneo. Hubo 7 hematomas peneano-escrotales, los cuales se resolvieron de forma espontánea. Conclusiones: La disfunción sexual fue la complicación más frecuente tras la uretroplastía. La anastomosis término-terminal es la técnica que menos complicaciones registra, mientras los colgajos fasciocutáneos ocasionan el mayor número. El empleo de mucosa bucal es un procedimiento con mínima morbilidad |
| English abstract | Aims: Reconstructive urethral surgery is the procedure that offers the best results, but it is not free from complications. We reviewed the complications in a group of patients, describing their treatment and prevention. Methods: one hundred and eighty-two patients that were diagnosed with urethral stricture and operated on were included in the study. We performed 95 end-to-end urethroplasties in the bulbar urethra; 40 urethroplasties with buccal mucosa graft, 30 of which were in the bulbar urethra in the ventral and dorsal position, and 10 in the penile urethra; 25 penile urethroplasties with fasciocutaneous lap; 13 perineostomies; and 9 posterior end-to-end anastomoses due to traumatic rupture of the urethra. Surgery was performed in two stages in 6 cases due to lichen sclerosus. The mean follow-up period was 48 months. Results: Twenty-three patients presented with some type of sexual dysfunction. Urethral diverticula were detected in 7 patients, 6 of which were treated with a repeat urethroplasty. There were 6 cases of postoperative istula that was resolved through conservative treatment in 4 cases and surgery in 2. Twenty patients presented with complications that required medical treatment: 5 surgical wound infections, 2 urinary infections, 4 cases of orchiepididymitis, one pulmonary embolism, and one case of neuropraxia of the peroneal nerve. There were 7 penoscrotal hematomas that resolved spontaneously. Conclusions: Sexual dysfunction was the most frequent complication resulting from urethroplasty. End-to-end anastomosis was the technique that had the fewest complications, whereas fasciocutaneous laps caused the greatest number of complications. Morbidity was minimal with the use of the buccal mucosa graft |
