Spanish abstract | El apoyo con nutrición parenteral total (NPT) es esencial para ayudar al crecimiento de los neonatos críticamente enfermos, pero debe iniciarse la vía enteral (IVE) a la brevedad. Objetivo: Analizar el impacto en el crecimiento ponderal del momento del inicio de la vía enteral en neonatos apoyados con NPT. Pacientes, materiales y métodos: Estudio de cohortes prospectivas. Neonatos admitidos en una unidad de cuidados intensivos neonatales (UCIN) con apoyo de NPT desde su nacimiento. Tres grupos con momentos distintos en el IVE (24-48 h; 49-72 h y 73-96 h). Se determinó la velocidad de crecimiento diario (g/kg) y el crecimiento acumulado a siete días post-IVE. Se comparó el crecimiento entre los grupos con ANOVA. Resultados: 113 neonatos, 84% pretérmino: 64 (57%) con IVE a las 24-48 h, 34 entre las 49 y 72 h (30.5%) y 14 entre las 73 y 96 h (12.5%). El promedio de velocidad de crecimiento fue mayor en los primeros (9.2 ± 2.1 g/kg/d versus 4.9 ± 2.3 g/kg/d versus 8.7 ± 3.7 g/ kg/d, respectivamente, p = 0.0007), así como el crecimiento acumulado (88.5 ± 12.9, 59.7 ± 13.7 y 63.8 ± 26.3 g/kg, p = 0.0003). Conclusiones: Iniciar la vía enteral lo más pronto posible en un neonato críticamente enfermo apoyado con NPT mejora el crecimiento ponderal |
English abstract | Support with total parenteral nutrition (TPN) is imperative to help the growth of critically ill neonates, but the enteral route should be started as soon as possible. Objective: To analyze the impact on weight growth of the time of the start of the enteral feeding (SEF) in neonates supported with TPN. Patients, material and methods: Prospective cohort study. Neonates admitted to a neonatal intensive care unit (NICU) with TPN support since birth. Three groups with different moments of SEF (24-48 h, 49-72 h and 73-96 h). The daily growth rate (g/kg) and the accumulated growth at seven days post-EF were determined. The growth between the groups was compared with ANOVA. Results: 113 neonates, 84% preterm: 64 (57%) with IVE at 24-48 h, 34 between 49-72 h (30.5%) and 14 between 73-96 h (12.5%). The average growth rate was higher in the fi rst group (9.2 ± 2.1 g/kg/d vs. 4.9 ± 2.3 g/kg/d vs. 8.7 ± 3.7 g/kg/d, respectively, p = 0.0007), as well as the accumulated growth at one week (88.5 ± 12.9, 59.7 ± 13.7 and 63.8 ± 26.3 g/kg, p = 0.0003).
