| Spanish abstract | El hipopituitarismo es una causa rara de talla baja y su diagnóstico requiere de un análisis clínico integral para lograr el tratamiento oportuno y completo. Se presenta el caso de un varón de 7 años, prematuro de 27 semanas, que acudió a consulta por talla de 100 cm (puntuación z [sz] de los Centers for Disease Control and Prevention [CDC]: -3.59) con talla diana 161.3 cm (sz de CDC: -2.16). El paciente presentó hipoplasia dental e hipotiroidismo central. Tras el inicio del tratamiento sustitutivo con levotiroxina no hubo mejoría en la talla. Se realizó la prueba de secreción inducida de hormona del crecimiento (GH) por hipoglucemia, que incrementó el GH y el cortisol a 4 ng/dL y a 17.9 µg/dL, respectivamente; simultáneamente se practicó una resonancia magnética (RM) que reportó hipófisis normal; con los resultados integramos el diagnóstico de fallo hipofisario múltiple e hipoplasia dental y probable síndrome de Rieger, por lo que se añadió al tratamiento GH recombinante, mejorando la velocidad de crecimiento en 1.25 cm/mes |
| English abstract | CDC: -3.59) and genetic stature of 161.3 cm (z score of CDC: -2.16). The patient presented dental hypoplasia and central hypothyroidism. There was not size improvement after the initiation with levothyroxine. The test of induced secretion of growth hormone (GH) by hypoglycemia raised GH and cortisol to 4 ng/dL and to 17.9 µg/dL, respectively . Simultaneously, a magnetic resonance imaging reported normal pituitary. The diagnosis of multiple pituitary failure and probable Rieger syndrome was integrated and the patient began treatment with recombinant GH, improving the growth rate in 1.25 cm/month |
