| Spanish abstract | Introducción: el neurilemoma o schwannoma mediastinal es una neoplasia benigna poco frecuente en el mediastino; aún más raro es que sea de gran dimensión. Es asintomático hasta que comprime algún nervio vecino o estructura vascular, pudiendo poner en peligro la vida. Caso clínico: hombre de 29 años de edad, con dolor en hemitórax izquierdo de dos meses de evolución, a quien se realizó radiografía de tórax, encontrando una opacidad en el lado izquierdo. Se le realizaron broncoscopia, tomografía axial computarizada y biopsia transtorácica dirigida. Fue sometido a toracotomía posterolateral izquierda. Conclusiones: la mayoría de las veces el neurilemoma mediastinal gigante no provoca síntomas, y cuando lo hace son inespecíficos. Se debe descartar tumor maligno, quistes o patología pulmonar no neoplásica, como colección pleural o intraparenquimatosa. El tratamiento es extirpar la tumoración por toracotomía posterolateral cuando son tumores de mediastino posterior o por esternotomía media en tumores de mediastino anterior. La toracoscopia videoasistida puede ser útil en la resección de tumores mediastinales pequeños, y en el abordaje diagnóstico cuando la punción transtorácica es negativa |
| English abstract | Background: mediastinal neurilemmoma is an infrequent and a big size neoplasm in the posterior mediastinum. There are usually no symptoms. When there is a pressure against nerves or vascular structures, it can be dangerous for the life. The main problem of this tumors is the diagnosis; surgical approach and the complete resection without injuring vital mediastinal structures. Clinical case: to report a case in a 29 year old male with a giant mediastinal neurilemmoma. He had chest pain two months. Chest rays showed opacity in the left chest. A broncofiberscopy, computed tomography and transthoracic biopsy were performed. Finally, the patient left posterolateral thoracotomy. Conclusions: the giant mediastinal neurilemmoma is a benign and asymptomatic neoplasm in most cases. When the symptoms are present, they are nonspecific. We should rule out other pathologies. The intraoperatory fiberscopy is useful for the resection of small mediastinal tumours |
