| Spanish abstract | Se realizó el reemplazo total de tres articulaciones temporomandibulares con el sistema de W. Lorenz en dos pacientes con anquilosis: la primera con anquilosis ósea, en la que había ausencia total de los movimientos mandibulares, condición que presentaba desde el año y medio de edad; la segunda con anquilosis fibrosa, por lo que presentaba limitación de la apertura oral de 12 mm, y condición posterior a fractura subcondílea no tratada, con tres años de evolución. La primera paciente fue sometida a reemplazo total de las dos articulaciones temporomandibulares mientras que en la segunda fue realizado solo el reemplazo de una articulación temporomandibular. El seguimiento, hasta el momento de este informe, es de ocho meses, con una apertura oral estable de 29 mm en el primer caso y de 34 mm en el segundo, sin desviación de la mandíbula durante la apertura ni dolor posoperatorio en ninguna de las dos pacientes. En ambos casos no hubo afectación del nervio facial de manera permanente. El reemplazo total de la articulación temporomandibular demostró ser efectivo y con resultados estables en pacientes con anquilosis de esa región |
| English abstract | Three total temporomandibular joint (TMJ) replacement surgeries were performed on two patients with TMJ ankylosis using W. Lorenz system. The first surgery was performed on a female patient with osseous ankylosis who had no mandibular movement since she was 18 months old. The second surgery was on a patient with fibrous ankylosis who had an interincisive opening distance of 12 mm. Her condition was the result of a three year old subcondylar fracture which didn’t receive any treatment. The first patient had total replacement of both TMJ’s, while the other just one TMJ. The follow up until today has been of eight months with an interincisive opening of 29 mm in the first patient and 34 mm in the other. There has not been any mandibular deviation during opening, nor postoperative pain in both cases. Facial nerve affection was not permanent in both patients. For all of this, we can conclude that total TMJ replacement is an effective treatment with stable results in patients with TMJ ankylosis |
