| Spanish abstract | Los opioides son la piedra angular en el tratamiento del dolor crónico de intensidad moderada a elevada, pero por su mecanismo de acción presentan ciertas particularidades que los hacen fármacos de riesgo. Sólo una minoría de pacientes con dolor crónico está en alto riesgo de desarrollar problemas de adicciones, considerando a la población entera de pacientes con dolor. De acuerdo a sus propiedades farmacológicas, los opioides presentan una afectación directa o indirecta a una vía nerviosa presente en el cerebro, que es el sistema de recompensa mesolímbico, lo cual los clasifica como fármacos de riesgo para adicciones. Pero en el paciente con dolor, sin un antecedente claro de abuso de otras sustancias desde tabaco, alcohol o cualquier droga, no hay riesgo para desencadenar una adicción durante su uso médico |
