Spanish abstract | El grupo de estreptococos Milleri está conformado por los microorganismos Streptococcus constellatus, Streptococcus intermedius y Streptococcus anginosus. Todos ellos son comensales habituales de la cavidad oral, y del tracto gastrointestinal y genital; sin embargo, llegan a ocasionar infecciones piógenas e invasivas que tienden a la formación de abscesos, bacteriemia y, en casos más graves, septicemia y choque séptico. Cuando no existen abscesos clínicamente aparentes, es necesario el aislamiento del microorganismo y realizar una búsqueda en profundidad para detectarlos. La presencia de hemocultivos positivos debe guiar la detección de una infección abdominal oculta, o bien, una infección supurativa focal o a distancia. En la presente carta se describe el caso de una mujer adolescente de 13 años de edad con bacteriemia por S. constellatus |
English abstract | The Milleri group of streptococci is made up of the microorganisms Streptococcus constellatus, Streptococcus intermedius and Streptococcus anginosus. All of them are habitual commensals in the oral cavity and in the gastrointestinal and genital tract; however, they can cause pyogenic and invasive infections that tend to the formation of abscesses, bacteremia and, in more severe cases, septicemia and septic shock. When there are no clinically apparent abscesses, it’s necessary to isolate the organism and perform an in-depth search to detect them. The presence of positive blood cultures should guide the detection of an occult abdominal infection, or a focal or distant suppurative infection.
