| Spanish abstract | En el expediente clínico quedan registradas las distintas actividades que implica el proceso de la atención médica, y la calidad de las instituciones de salud se reeja entre otros aspectos en la calidad de su expediente clínico. Objetivo: Determinar el nivel de calidad del expediente clínico de pacientes adultos programados para cirugía de acuerdo con el Modelo de Evaluación del Expediente Clínico Integrado y de Calidad (MECIC) en una unidad hospitalaria de 3er Nivel de Atención. Material y Métodos. Estudio cuantitativo, correlacional y retrospectivo, de los servicios de Cirugía General, Neurocirugía y Ortopedia. El instrumento que corresponde al MECIC está integrado por 16 dominios. Resultados. 3/4 partes de los expedientes cumplen con los requerimientos de acuerdo con el MECIC; en su análisis por dominio ninguno alcanzó un nivel de cumplimiento de 90% o más; el promedio general fue de 70.4%. De los 12 dominios, 6 alcanzaron un nivel superior al 80%, éstos fueron: Nota de Referencia, Trabajo Social, Nota de Urgencias, Nota Posanestésica, Consentimiento Informado y Nota de Egreso. Con un cumplimiento de 70% fueron: Nota Preanestésica, Historia Clínica, Nota Preoperatoria, Servicios Auxiliares de Diagnóstico y Tratamiento Médico, Notas Médicas y la Hoja de Enfermería. Los Dominios que se encuentran en 60% o menos de cumplimiento: Generalidades del Expediente, Nota de Interconsulta y Registro de Transfusiones. Conclusiones. El nivel de cumplimiento de los expedientes clínicos se encuentra en una categoría de regular. Sí existen diferencias en el cumplimiento del expediente por servicio; de acuerdo con el MECIC, Ortopedia tuvo 65% de excelencia, Neurocirugía 32% bueno y Cirugía General 82% fueron insucientes. | English abstract | The different activities that consist of health care are registered in the medical chart; as well as the quality of the medical institutions is reected, among other aspects, in the quality of the medical chart. Objective: To determine the quality level of the medical chart of adult patients scheduled for surgery according to the Evaluation Model of the Integrated Medical Chart and of Quality (MECIC in Spanish) in a hospital unit at the third Attention Level. Material and methods. A quantitative, correctional and retrospective study on the services of General Surgery, Neurosurgery and Orthopaedics. The MECIC suitable instrument is integrated by 16 elds. Results. Three out of four parts of the charts full the MECIC requirements. In their eld analyses none achieved a 90% or more compliance level; the general level was 70.4%. Out of 12 elds, six managed a superior level of 80%, these were: Reference Note, Social Work, Urgency Note, Recovery Note, Informed Consent and Discharge Note. The elds with 70% compliance were: Pre-anaesthetic Note, Medical History, Pre-surgery Note, Auxiliary Service of Diagnostic and Medical Treatment, Medical Notes and Nursing Note. The elds that have a compliance level of 60% or less are: Records Generalities, Consultation Note and Transfusions Registry. Conclusions. The level of compliance of the medical charts is steady. There are differences in the compliance of the service chart; according to the MECIC, Orthopaedics had 65% of “excellence”, Neurosurgery 32% of “good” and General Surgery 82% of “insufcient”. The Model of Evaluation of the Integrated Medical Chart and of Quality is a tool that allows the homogenization of the evaluation system of the medical chart |
