| Spanish abstract | OBJETIVO: Reportar una modalidad de atención a las pacientes con acretismo placentario en sus diferentes grados y detallar los pasos a seguir a partir del momento del diagnóstico, la planeación quirúrgica, la cirugía y los cuidados posoperatorios, y lo que con ello se ha conseguido. MATERIALES Y MÉTODOS: Estudio retrospectivo, de revisión de expedientes de pacientes con acretismo placentario efectuado en el Hospital San José de Monterrey, por el servicio de Obstetricia de ISSSTELEON, entre julio de 2017 y mayo de 2018. Se analizaron, mediante estadística descriptiva, el tiempo quirúrgico, sangrado transoperatorio, hemoglobina pre y posoperatorio, días de internamiento, necesidad de ingresar a cuidados intensivos y complicaciones transquirúrgicas. Se describen los pasos efectuados por los equipos de Obstetricia y el multidisciplinario de apoyo. RESULTADOS: Se practicaron 4 histerectomías, con disminución del sangrado trans-operatorio, no se aplicaron paquetes globulares y el alta hospitalaria se dio entre el tercero y sexto días del posoperatorio. No hubo reinternamientos ni reintervenciones en el periodo puerperal; 3 de las 4 pacientes pasaron a cuidados intermedios en las primeras 24 horas, para observación. No fue necesario ingresar a las pacientes a terapia intensiva. Durante los procedimientos, los recién nacidos permanecieron en cuidados neonatales, sin complicaciones. CONCLUSIONES: El tratamiento de estas pacientes siempre ha representado un gran reto, por la alta morbilidad quirúrgica. En este reporte se expone una innovación en la manera de tratar a estas pacientes, sustentada en la planeación preoperatoria y en la participación multidisciplinaria de distintos especialistas, con desenlaces favorables |
| English abstract | OBJECTIVE: Describe step by step, the surgical approach of obstetric hysterectomy that we are doing in our center for the treatment of patients with placental acretism. MATERIALS AND METHODS: Retrospective study of the file review of 4 cases of pregnant patients with placental accreta, performed at the San José de Monterrey Hospital, by the ISSSTELEON obstetrics service, conducted between July 2017 to May 2018. They were analyzed using descriptive statistics surgical time, trans operative bleeding, pre and post operative hemoglobin, days of hospitalization, transition to therapy, trans surgical complications. A description was also made of the steps taken by the obstetrics team and the multidisciplinary support team. RESULTS: 4 hysterectomies were performed, obtaining decrease in trans operative bleeding, no globular packages were used; discharge from the 3rd to 6th day post surgery. There were no re-hospitalizations or re-interventions in the puerperal period; three of the 4 patients went to intermediate care for the first 24 hours for observation. Patients were not transferred to intensive care. During the procedures the newborns went to neonatal care without complications. CONCLUSIONS: The approach of these patients has always represented a great challenge, due to their high morbidity and surgical morbidity. In this work we show an innovation in the approach of these patients, which has its cornerstone in pre-operative planning and multidisciplinary work of different specialties, with favorable results in the management and prognosis of these patients |
