| Spanish abstract | Con los términos de "benévola neutralidad", "neutralidad afectiva" y "separación compasiva", a los que pueden añadirse "universalismo" y "determinación funcional", se ha dado en llamar la postura del médico ante el paciente terminal o moribundo. En la asistencia al paciente terminal, el médico ha de intentar descubrir qué significan los síntomas para el paciente, cómo los experimenta, cuáles son los fenómenos resultantes de la enfermedad y cuáles son las molestias que siente a causa de éstos. En este estadio de la enfermedad ya no se trata de curarla, sino de hacer llevadero y soportable el sufrimiento derivado del mismo proceso nosiógeno. Esto significa que los síntomas deben ser no objetivados sino subjetivizados por el propio paciente y no por el mismo médico, ya que sus quejas pueden cobrar diversos significados. Escuchar y comprender son los más apropiados procedimientos terapéuticos que se han de emplear; la compasión deberá ser valorada en su propio derecho y no considerada como una cualidad extra para aquél que la posee: la capacidad para funcionar bien a pesar de los diversos problemas empieza con el reconocimiento de que la muerte es inevitable para todos y que el temor a ella es normal y aceptable |
| English abstract | With the terms of “benevolent neutrality", “affective neutrality” and “compassionate separation", to those that "universal" and “functional determination” can be added, it has been given in calling the posture of the doctor before the terminal or moribund patient. In the attendance to the terminal patient, the doctor has to try to discover that they mean the symptoms for the patient, how it experiences them which are the resulting phenomena of the illness and which are the nuisances that it feels because of these. In this stadium of the illness it is no longer of to cure it, but to make bearable and bearable the derivative suffering of the same process of illness. This means that the symptoms should be no to objective but to subjective for the own patient and no for the same doctor, since their complaints can charge sundries meanings. To listen and to understand is the most appropriate therapeutic procedures to use; the compassion will be valued in its own right and no considered as an extra quality for that that possesses it; the capacity to work well in spite of the diverse problems begins with the recognition of that the death is unavoidable for all and that the fear to the death is normal and acceptable |
