| English abstract | Objective: To evaluate left ventricular hypertrophy diagnosed with transthoracic echocardiography and its relationship with electrocardiographic changes. Methods: An analytical observational study was carried out in patients treated by the cardiology service at the General Hospital of Barranquilla MiRed IPS, from October 2020 to March 2021. Results: From a total of 111 patients, 63.1% (n = 70) were female, the mean age was 64.1 ± 11.8; 21.6% (n = 24) had hypercholesterolemia, 26.1% (n = 29) took 3-hour weekly walks, and 44.1% (n = 49) were obese. 90.1% (n = 100) received treatment, being losartan the most widely used with 52.3% (n = 58), 77.5% (n = 86) complied with the treatment, and 67.6 % (n = 75) showed normal creatinine values 0-1.0 mg / dL. A significant difference was established between the categories of the electrocardiographic patterns, 39.0% (n = 30) with left ventricular hypertrophy had poor R wave progression in V3 (p <0.000), and those who did not show ventricular hypertrophy left, had other electrocardiographic findings 67.6% (n = 67), (p <0.000). The patients with concentric hypertrophy pattern corresponded to 76.6% (n = 59), (p <0.000) those with eccentric hypertrophy had the electrocardiographic record with Sokolow-Lyon index 16.2% (n = 18). None of the electrocardiographic findings was sufficiently accurate in estimating left ventricular hypertrophy. Conclusion: Low sensitivity in the electrocardiographic indices for the diagnosis of left ventricular hypertrophy is reported in this study, with a great difference in its diagnostic power compared to echocardiogram. |
| Spanish abstract | Objetivo: Evaluar la hipertrofia ventricular izquierda diagnosticada con ecocardiograma transtorácico y su relación con los cambios electrocardiográficos. Métodos: Se realizó un estudio observacional analítico, en los pacientes atendidos por el servicio de cardiología en el Hospital General de Barranquilla MiRed IPS, durante octubre de 2020 a marzo de 2021. Resultados: De un total de 111 pacientes, el 63,1% (n=70) correspondieron al sexo femenino, la edad promedio fue de 64,1 ± 11,8; el 21.6 % (n=24) presentaban hipercolesterolemia, el 26.1% (n=29) realizaban caminatas 3h semanales, y el 44.1% (n=49) eran obesos. El 90,1% (n=100) recibían tratamiento, siendo el losartan el de mayor uso con un 52,3% (n=58), el 77.5% (n=86) cumplía con el tratamiento, y el 67,6% (n=75) mostro valores normales de creatinina 0-1,0 mg/dL. Se estableció una diferencia significativa entre las categorías de los patrones electrocardiográficos, el 39,0% (n=30) con hipertrofia ventricular izquierda presentaron una pobre progresión de la onda R en V3 (p< 0.000), y aquellos que no mostraron hipertrofia ventricular izquierda, tenían otros hallazgos electrocardiográficos 67,6% (n=67), (p< 0.000). Los pacientes con patrón de hipertrofia concéntrica correspondieron el 76,6% (n=59), (p< 0.000) aquellos con hipertrofia excéntrica tenían el registro electrocardiográfico con índice de Sokolow-Lyon 16,2% (n=18). Ninguno de los hallazgos electrocardiográficos fue lo suficientemente exacto en la estimación de la hipertrofia ventricular izquierda. Conclusión: Se reportan en este estudio baja sensibilidad en los índices electrocardiográficos para el diagnóstico de hipertrofia ventricular izquierda con una gran diferencia en su poder diagnostico respecto al ecocardiograma. |
