| Spanish abstract | El estado "dippers" es un factor de riesgo independiente en los pacientes hipertensos ("Risers"-R->"Non-dippers"-ND->"Dippers"-D>"Dippers extremos"-DE). Se evaluó el efecto de la nueva nifedipina en microgránulos (NMG) una vez al día en la disminución de la presión arterial nocturna medida a través del MAPA con Mobil-o-Graph -CE0434-(I.E.M.Gmbh-Cockerillstr. 69 D-Stolberg. Germany). Métodos: Se incluyeron pacientes hipertensos (PAS ≥ 140 y/o PAD ≥ 90 mmHg, medidas con esfigmomanómetro de Hg) que recibieron de 30 a 60 mgrs/día de NMG, en un estudio prospectivo abierto comparativo y cruzado en "time-doses" 8 am ó 8 pm; que luego de 6 semanas de tratamiento mantuvieran PAS < 140 mmHg. Resultados: Se reclutaron 73 pacientes. 40 pacientes (54.8%), mantuvieron su estado: ND n = 22 (disminución de la PAS nocturna entre 0 y 10%), D n =16 (disminución de PAS nocturna entre 10 y 20%) y R n = 2 (incremento de la PAS nocturna). Los otros 33 pacientes (45.2%) modificaron su estado: cinco: 3D y 2ND cambiaron a DE (disminución de la PAS nocturna >20%), once: 2DE y 9ND se transformaron en D, catorce: 6R, 1DE y 7D hacia ND y 3D cambiaron a R. No hubo diferencias significativas en el cambio de estado "dippers" en ambos grupos (30 ó 60 mgrs), 17 pacientes mejoraron su estado (9ND y 2DE) se transformaron en D y 6R pasaron a ND; 16 pacientes "dippers" se mantuvieron. Conclusión: Estos resultados sugieren que la NMG ofrece en el 45% de los pacientes estudiados un beneficio independiente de la reducción de la presión arterial, manteniendo o mejorando el estado "dippers" ofreciendo una mejor predicción en la disminución de eventos cardiovasculares y del pronóstico de los accidentes cerebro-vasculares |
| English abstract | The dipper status is an independent risk factor in the hypertensive patients (Risers-R->Non dipper-ND->Dipper-D->Extreme dipper-ED-). We evaluated the effect of new nifedipine once day in micro-granules (NMG) on the nocturnal blood pressure fall, measurement by ABPM. Methods: Were included hypertensive patient (SBP ≥ 140 mmHg and/or DBP ≥ 90 mmHg, measurement with sphygmomanometer) that received 30 o 60 mg/day in a prospective, open and crossed in the time-doses 8 am or 8 pm study and that after the six week of treatment if SBP were < 140 mmHg with. Results: We recruited 73 patients, 40 (54.8%) maintained their status: ND n = 22 (night SBP fall between 0 and 10%), D n = 16 (night SBP fall between 10 and 20%) and R n = 2 (nocturnal increase in SBP). The other 33 patients (45.2%) change their status, five: 3 D and 2 ND change to ED (night SBP fall > 20%), eleven: 2 ED and 9 ND change to D, fourteen: 6 R, 1 ED and 7 D changed to ND, and three: 3 D change to R. There were no statistical difference with nocturnal blood pressure dipping change among the groups (30 or 60 mg), 17 patients improved their status (9 ND and 2 ED to D and 6 R to ND) and 16 patients D, maintained it. Conclusions: These results suggest that NMP offers in the 45% of the patients maintained or improve the status like as an independent benefit for the reduction of the blood pressure that could result in a better prediction of cardiovascular events and influence the prognosis for stroke |
