Spanish abstract | La hipertensión arterial pulmonar es una enfermedad progresiva y fatal para la cual no existe un tratamiento curativo. En los últimos años ha ocurrido un avance muy grande en el conocimiento de los mecanismos íntimos del daño vascular característico de la enfermedad. Este conocimiento ha evolucionado desde la vasoconstricción originalmente planteada, hasta el concepto de proliferación (fibromuscular) anormal en la estructura vascular que tiene mayor preponderancia actual. Esta evolución conceptual, ha generado intervenciones farmacológicas que pretenden interrumpir el proceso de proliferación y remodelación vascular anormal característica de la enfermedad. En esta revisión se analizan las intervenciones derivadas del conocimiento patobiológico, y se establece una estrategia terapéutica basada en la evidencia obtenida de los más recientes ensayos clínicos controlados |
English abstract | Pulmonary arterial hypertension is a progressive and lethal disease for which there is no effective cure. In recent years, however, there has been a significant progress in the knowledge about the pathobiologic mechanisms involved in the characteristic vascular damage of this disease. Knowledge has evolved from the importance of the vasoconstriction component originally contemplated to the current concept of a preponderant abnormal (fibromuscular) proliferation. This conceptual evolution has resulted in new and attractive pharmacologic interventions in an attempt to interrupt this abnormal process of vascular remodeling. In this review, the interventions derived from this pathobiologic knowledge are analyzed and based on the most recent controlled clinical trials; an evidence–based therapeutic strategy is established |
