| Spanish abstract | La enfermedad de Parkinson es uno de los desórdenes neurodegenerativos más importantes por su creciente prevalencia y la gran limitación en las actividades de la vida diaria que produce. La progresión de esta enfermedad afecta la capacidad motora de los pacientes y, aunque las posibilidades terapéuticas se han incrementando en los últimos 40 años, las complicaciones que producen limitan su utilidad y eficacia en poco tiempo. La posibilidad de un tratamiento quirúrgico con el uso de estimulación cerebral profunda ha permitido no sólo ampliar el horizonte de tratamiento, sino contrarrestar las complicaciones del tratamiento médico e incrementar la capacidad funcional de los pacientes. En nuestro país, sólo algunos centros de tercer nivel han integrado equipos especializados para el uso de esta nueva opción de manejo. Presentamos el primer caso tratado en el Centro Neurológico del Centro Médico ABC con estimulación cerebral profunda del núcleo subtalámico en un paciente con enfermedad avanzada y discinesias incapacitantes, el cual manifestó una recuperación clínica muy importante que le permitió recuperar sus actividades rutinarias con una excelente calidad de vida. Se describen las características técnicas del procedimiento, los criterios de selección de los pacientes y la experiencia clínica en diferentes centros alrededor del mundo |
| English abstract | Parkinson’s disease is one of the most important neurodegenerative disorders because it’s increasing prevalence, but mainly it’s in relation with the major limitation in activities of daily life. The progression of this disease affects the motor skills of patients, and although the therapeutic possibilities have been increasing in the last 40 years, the onset of drug induce complications limit its usefulness and effectiveness in the short time. The possibility of a surgical treatment using Deep Brain Stimulation has allowed broaden the treatment horizon, limit the drug induce complications, and increase the functional capacity of the patients. In our country only few tertiary centers have specialized teams for the use of this new treatment option. We report the first case treated at the Neurological Center of the ABC Medical Center with Deep Brain Stimulation of the subthalamic nucleus in a patient with advanced disease and disabling dyskinesias that obtained an important clinical improvement to recover his routine activities with excellent quality of life. With this case presentation we describe the technical features of the procedure, main criteria for patient selection and the clinical experience in different centers around the world |
