| Spanish abstract | Una complicación difícil para la cirugía ortopédica es la seudoartrosis y la cadera no está exenta de ello, representa un verdadero problema, sobre todo cuando se presenta en pacientes jóvenes, sin embargo en la actualidad la cirugía de reemplazo articular brinda ventajas para dar mejor calidad de vida. Objetivo. Presentar la reconstrucción articular temprana con reemplazo total de cadera con prótesis de sustitución de calcar, en pacientes tratados con seudoartrosis de cadera para su rehabilitación temprana. Material y métodos. Se presentan dos casos de seudoartrosis de la cadera tratados y sin buenos resultados con osteosíntesis y en los cuales sólo el reemplazo total con un componente femoral con sustitución de calcar solucionaba el problema. Caso uno: femenino 37 años con seudoartrosis proximal de fémur y pérdida ósea se colocó prótesis total no cementada y se colocó injerto autólogo para reconstruir fémur proximal. Caso dos: masculino de 55 años diabético con seudoartrosis de la cadera, se colocó prótesis total no cementada con sustitución de calcar. Resultados. Los dos pacientes presentaron movilidad temprana, apoyo parcial con ayuda de andadera a las dos semanas y apoyo total a las 6 semanas; fuerza muscular íntegra a las 4 semanas, con reintegración a sus actividades a las 10 semanas. Conclusiones. El reemplazo total de cadera es un método de rescate en pacientes con calidad de vida pobre que han tenido complicaciones y que no hay opción de tratamiento, sin embargo no es la solución definitiva a su problema, pero sí les brinda mejor calidad de vida |
| English abstract | A difficult complication for the orthopaedic surgery is the pseudoarthrosis and the hip not this exempt of it represents it mainly a true problem when it is presented in young patients; however at the present time the substitution surgery to articulate offers advantages to give better quality of life. Objective. To present the reconstruction to articulate early with total substitution of hip with substitution prosthesis of calcar, in patients treated with pseudoarthrosis of hip for their early rehabilitation. Material and methods. Two cases treated of pseudoarthrosis of hip are presented, without good results with osteosynthesis in those which alone the total substitution with a femoral component with substitution of calcar solved the problem. Case one: feminine 37 years with pseudoarthrosis femoral proximal and lost bony, which treated with total prosthesis not cemented and bone graft autologous to reconstruct femur proximal, Case two: masculine of 55 years diabetic with pseudoarthrosis of hip which treated with total prosthesis not cemented with substitution of calcar. Results. The two patients presented early mobility, partially support with the help mechanical device to the two weeks and total support to the 6 weeks muscular force it integrates to the 4 weeks, with reinstatement to their activities to the 10 weeks. Conclusions. The total substitution of hip is a rescue method in patient with quality of poor life that they had complications and that there is not solution to its problem but if it offers better quality of life |
