Spanish abstract | El tumor de células gigantes representa 5% de todos los tumores óseos primarios y 20% de las neoplasias óseas benignas. Las localizaciones más comunes son el fémur distal y la tibia proximal (50%) y el radio distal (10%). Los métodos de tratamiento incluyen la resección intralesional para los tumores latentes y activos, y la resección amplia para las lesiones agresivas. La reconstrucción de la muñeca posterior a una resección amplia del radio distal constituye un reto para el cirujano ortopedista. Objetivo: La presentación de 2 casos clínicos de pacientes diagnosticados con tumor de células gigantes del radio distal que fueron tratados con resección amplia y colocación de aloinjerto masivo con artrodesis de la muñeca. Realizar una revisión bibliográfica y el análisis de los diferentes métodos de tratamiento descritos. Material y métodos: Descripción del tratamiento y evolución presentada en cada caso, así como la revisión bibliográfica y el análisis de los tratamientos encontrados. Resultados: Datos radiológicos de integración del aloinjerto a los 11 meses y funcionalidad compatible con todas las actividades de la vida diaria. Discusión: El aloinjerto masivo del radio distal con artrodesis de la muñeca representa una muy segura y buena opción para la reconstrucción de ese segmento anatómico posterior a una resección amplia |
English abstract | The giant cell tumor represents 5% of all the primary bone tumors and 20% of the benign bone neoplasias. The most common locations are the distal femur and the proximal tibia (50%) as well as the distal radius (10%). Treatment methods include the intralesional resection of the latent and active tumors, and broad resection for the aggressive lesions. The wrist reconstruction after broad resection of the distal radius represents a challenge for the orthopedic surgeon. Objective: To present 2 clinical cases of patients diagnosed with giant cell tumor of the distal radius who were treated with broad resection and placement of a massive allograft with wrist arthrodesis. To perform a bibliographic review and the analysis of the different treatment methods described. Material and methods: Description of the treatment and course of each case, as well as a bibliographic review and the analysis of the treatments found. Results: Radiologic data of the allograft integration at 11 months and a functionality which was compatible with all the activities of daily living. Discussion: The massive allograft of the distal radius with wrist arthrodesis represents a very safe and appropriate option for the reconstruction of this anatomic segment after broad resection |
