Resumen en español | La presentación clínica de las hepatitis virales muestra características similares independientemente del agente causal, por lo que es fundamental establecer un diagnóstico etiológico específico, del que dependerá en gran medida el pronóstico del caso en particular. Unido a esto, existen gran número de pruebas diagnósticas, de compleja interpretación e interrelación secuencial. Debido a esto, los autores, después de una exhaustiva revisión bibliográfica, proponen un algoritmo para facilitar el diagnóstico etiológico de esta patología. En el mismo se enfatiza: 1.- Existencia de virus hepatotrópicos (VHA, VHB, VHC, VHE) y no hepatotrópicos (VEB y CMV) como agentes etiológicos. 2.- Se señala la secuencia a seguir dependiendo de los resultados obtenidos en las pruebas diagnósticas sugeridas previamente. 3.- Se proponen como pruebas diagnósticas iniciales en todo paciente con sospecha clínica de hepatitis viral aguda: IgM anti VHA, AgHBs, IgM anti HBc. 4.- La negatividad de estas pruebas iniciales obliga al descarte de HVC.. Adicionalmente se hace una revisión actualizada de la literatura en relación a las pruebas diagnósticas existentes y su interpretación |
Resumen en inglés | Clinical presentation of MN viral hepatitis shows similar characteristics regardless of the causal agent, for what it is fundamental to establish a specific diagnosis, on which the prognostic of the case will depend. Moreover, there exist great number of diagnostic tests, of complex interpretation and sequential interrelationship. Consequently, after a thorough literature review, the authors propose a flow diagram to facilitate the diagnostic of a specific agent of this pathology which emphasizes: 1. - Existence of some virus with hepatic tropism (HAV, HBV, HCV, and HEV) and some with no hepatic tropism (EBV and CMV) as specifics agents. 2.-The sequence is indicated depending on the results obtained in the diagnostic tests previously suggested. 3.- The initial tests proposed for every patient with any clinical suspicion of acute viral hepatitis: IgM anti HAV, HBsAg, IgM anti HBc. 4.- Negative results in the initial tests will rule out VHC. Additionally, an update review of the literature on existing diagnostic test and its interpretation is presented |
