Resumen en español | Comparar velocidad de crecimiento ponderal y factores asociados en neonatos pretérmino de 28 a 34 SEG, según peso al nacer, durante su estancia en la UCIN. Pacientes: dos cohortes, peso adecuado (PA, n = 64) y bajo peso para edad gestacional (BP, n = 21). Mediciones: velocidad de crecimiento ponderal semanal (g/d) ajustado por: sexo, SEG, días ayuno, días nutrición parenteral, uso de aminas, asistencia ventilatoria y sepsis. Análisis estadístico: t Student, χ2 y análisis de regresión lineal múltiple. Resultados: En la primera semana, ambos grupos bajaron de peso (velocidad PA: -2.9 contra BP: -1.5 g/d, p = 0.21). En las semanas dos a siete el incremento ponderal fue en promedio > 20 g/d (p = ns). A la semana ocho sólo los PA redujeron su velocidad (13.8 contra 30.7, p = 0.03). Diecisiete punto seis por ciento. (15/85) egresaron con peso bajo para su edad corregida, principalmente los de BP (RR = 4.57, IC 95% 1.8 a 11.3, p = 0.001). La NPT infl uyó positivamente (beta = 3.1 g/d), contrario a la ventilación mecánica (B = -33.3 g/dL). Conclusión: En todos los prematuros la velocidad de crecimiento fue negativa la primera semana y posteriormente revirtió a una positiva. A partir del mes los BP aceleraron su crecimiento |
Resumen en inglés | To compare weight gain velocity (WGV) and its associated factors in preterm newborns (28 to 34 WGA), according to their birth-weight for gestational age, during their stay at the NICU. Patients: two cohorts; on with adecuate birth-weight (PA, n = 64) and other with low birth-weight (BP, n = 21). Measurements: weekly WGV (g/d) adjusted by: sex, weeks of gestation, days of fasting, days with parenteral nutrition, use of amines, ventilatory support and presence of sepsis. Statistical analysis: t Student, χ2 test and multiple linear regression analysis. Results: During the fi rst week, both groups reported weight loss (WGV PA -2.9 versus BP -1.5 g/d, p = 0.21). From the second to seventh week, WGV were in average > 20 g/d (p = ns). At the eight week only PA group reduced their growth velocity (13.8 versus 30.7, p = 0.03). At discharge, 17.6% (15/85) was in low weight for their corrected gestational age, mostly the BP group (RR = 4.57, IC 95%, 1.8 to 11.3, p = 0.001). Parenteral nutrition had a positive infl uence in WGV, contrary with the ventilatory support. Conclusion: In all preterm infants the rate of intrahospital growth is negative the fi rst week and reverts in the second one. From month one, the BP accelerates more its growth |
