Resumen en español El sarcoma epitelioide de tipo proximal es una variante infrecuente del sarcoma epitelioide convencional descrito en pacientes de edad avanzada en la porción proximal de las extremidades, con predilección por la pelvis y la zona perineal. Clínicamente es más agresivo, muestra una mayor incidencia de recurrencias, metástasis, resistencia a la quimioterapia y mortalidad más elevada. Describimos el caso de una paciente con sarcoma epitelioide de tipo proximal en pubis y revisamos la literatura precedente. Caso clínico: Mujer de 57 años de edad con una masa indolora en pubis, de crecimiento progresivo. Se realizó la exéresis quirúrgica de la lesión con márgenes libres y el estudio histológico e inmunohistoquímico permitió el diagnóstico definitivo de sarcoma epitelioide de tipo proximal. Después de cuatro años libres de enfermedad, la paciente presentó recidiva tumoral en la misma localización. Se realizó exéresis en bloque de la lesión y el estudio inmunohistoquímico confirmó la recidiva. La paciente rechazó el tratamiento adyuvante. Después de cuatro años de seguimiento, la paciente permanece asintomática y sin datos de recidiva o enfermedad a distancia. Conclusiones: El diagnóstico del sarcoma epitelioide de tipo proximal es complejo pues histológicamente puede confundirse con múltiples tumores, por ello el estudio inmunohistoquímico resulta definitivo. Los sarcomas epitelioides de tipo proximal muestran positividad para marcadores epiteliales (citoqueratinas y antígeno de membrana epitelial), marcadores mesenquimáticos (vimentina), así como para CD34. El tratamiento quirúrgico con márgenes libres junto con terapias adyuvantes cuando el riesgo de recurrencia es elevado, constituye la práctica más referida en la literatura
Resumen en inglés The proximal-type epithelioid sarcoma (PES) is a rare variant of conventional epithelioid sarcoma. It has been described in older patients in the proximal portion of the extremities with a predilection for the pelvis and perineum. It is clinically more aggressive, showing a higher incidence of recurrence, metastasis, resistance to chemotherapy and a higher mortality. We report the case of a patient with a PES of the pubic region and we review the previous literature. Clinical case: We report the case of a 57-year-old female with a painless, progressively growing mass in the region of the pubis. We performed surgical resection of the lesion with clear margins and histological and immunohistochemical study allowed the definitive diagnosis of PES. After 4 disease-free years, the patient had a tumor recurrence at the same location. We performed en bloc resection of the lesion and immunohistochemical study confirmed the recurrence of PES. The patient refused adjuvant therapy. Today, after 4 years of follow-up, the patient remains asymptomatic and without evidence of recurrence or distant disease. Conclusions: Diagnosis of PES is complex because histological findings may be confused with multiple tumors; therefore, immunohistochemical study is definitive. PES shows positivity for epithelial markers (cytokeratin and EMA), mesenchymal markers (vimentin) as well as CD34. According to what has been reported in the literature, surgical treatment with free margins is indicated, with adjuvant therapies when the risk of recurrence is high
