Resumen en español | El desfibrilador automático implantable (DAI) es el tratamiento de elección para la prevención primaria y secundaria de la muerte súbita (MS). En el Instituto Nacional de Cardiología, se han colocado 26 DAI en 25 pacientes de octubre de 1996 a enero del 2002. Hubo 23 hombres (92%) y la edad media del grupo fue de 51.4 años; 23 pacientes (92%) tuvieron cardiopatía estructural, la más frecuente fue la cardiopatía isquémica en 13 pacientes (52%) con un valor medio de la fracción de expulsión (FE) de 37.8%. Un paciente sin cardiopatía estructural tenía síndrome de Brugada. La arritmia clínica más frecuente fue taquicardia ventricular monomórfica sostenida (TVMS) en 14 pacientes (56%). El seguimiento de estos pacientes tuvo un valor medio de 29.3 meses; durante este período se registraron 30 terapias aplicadas por el DAI, 6 de ellas fueron inapropiadas por fibrilación atrial paroxística (FAP). En 10 DAI (34%) no se ha aplicado ninguna terapia. Ha habido tres muertes (12%) por insuficiencia cardíaca refractaria a tratamiento farmacológico. Conclusiones: La implantación del DAI es una medida segura y efectiva para la prevención primaria y secundaria de la MS. En la experiencia mundial, este dispositivo no ha modificado la mortalidad por insuficiencia cardíaca de estos enfermos, pero si la MS arrítmica |
Resumen en inglés | The automatic implantable defibrillator (AID) is the treatment of choice for primary and secondary prevention of sudden death. At the Instituto Nacional de Cardiología, since October 1996 until January 2002, 25 patients were implanted with 26 AID. There were 23 men (92%) and the mean age of the whole group, was 51.4 years. Twenty-three patients (92%) presented structural heart disease, the most common was ischemic heart disease in 13 patients (52%), with a mean ejection fraction of 37.8%. One patient without structural heart disease had Brugada Syndrome. The most frequent clinical arrhythmia was ventricular tachycardia in 14 patients (56%). The mean follow-up was of 29.3 months during which a total of 30 events of ventricular arrhythmia were treated through AID; six of them were inappropriate due to paroxismal atrial fibrillation; 10 AID patients (34%) have not applied for therapy. Three patients (12%) of the group died due to congestive heart failure refractory to pharmacologic treatment. Conclusion: The implant of the AID is a safe and effective measure for primary and secondary prevention of sudden death. World-wide experience evidences, that this kind of device has not modified the mortality rate due to heart failure in these patients, but it has diminished sudden arrhythmic death |
