Resumen en español | La infiltración grasa es un cambio degenerativo que disminuye el grosor y la fuerza del músculo. Tras una reparación del manguito de los rotadores con infiltración grasa, la tasa de falla es de 25%. Existen mediciones relacionadas con la severidad de la lesión del manguito rotador. Objetivo: Determinar qué mediciones están relacionadas con la infiltración grasa. Material y métodos: Se realizó un estudio retrospectivo, observacional, descriptivo y transversal. Se valoraron 133 pacientes con rotura del manguito de los rotadores para encontrar el grado de infiltración grasa del supraespinoso, el tamaño de rotura, la infiltración grasa del infraespinoso/redondo menor, la distancia acromio-humeral y el índice de Thomazeau. Se calculó el coeficiente de correlación y se realizó un análisis de regresión lineal múltiple. Resultados: El tamaño de rotura y el índice de Thomazeau tienen una correlación positiva fuerte con la infiltración grasa del supraespinoso. Se observó una correlación positiva moderada con la infiltración grasa del infraespinoso/redondo menor y una correlación negativa débil con la distancia acromio-humeral. Conclusiones: Se concluyó que la infiltración grasa del supraespinoso tiene correlación con la infiltración grasa del infraespinoso y el redondo menor, sin existir efecto estadísticamente significativo. La distancia acromio-humeral carece de efecto estadísticamente significativo. La infiltración grasa del supraespinoso está directamente relacionada con el tamaño de la rotura. La atrofia muscular y la infiltración grasa tienen una correlación estadísticamente significativa entre ambas, pero no debe considerarse el mismo proceso degenerativo, ya que generan pronósticos diferentes |
Resumen en inglés | Fatty Infiltration is a degenerative condition in the muscle that affects its width and strength. After a rotator cuff tear repair in a muscle with fatty infiltration there is a 25% chance of failure rate. Fatty infiltration is a standard value to predict the outcomes after a rotator cuff repair. Objective: The aim of this study is to determine which measurements are directly related to fatty infiltration and it’s outcome after a rotator cuff repair. Material and methods: 133 shoulder Magnetic resonance images were assessed to determine the tear size, fatty infiltration of the supraspinatus, fatty infiltration of the infraspinatus/ teres minor, acromio-humeral distance and Thomazeau index. A Pearson correlation index was calculated and a linear multiple regression analysis was performed to evaluate the variable influence of the fatty infiltration in the supraspinatus. Results: A strong positive correlation was found between the tear size and the fatty infiltration of the supraspinatus as well as the Thomazeau occupancy index. The infraspinatus/teres minor fatty infiltration has a moderate positive correlation with the supraspinatus fatty infiltration. acromiohumeral-distance has negative weak correlation between the fatty infiltration of the supraspinatus. Conclusions: Fatty infiltration of the infraspinatus/teres minor and acromio-humeral distance have non-statistically significance in relation with fatty infiltration. Tear size and the Thomazeau occupancy index had a p-value of statistical significance so they are related to the fatty infiltration of the supraspinatus, but muscular atrophy and fatty infiltration have different etiologies so they have a different influence on the outcome |
