Resumen en inglés Objectives: to establish the security of the employ of an percutaneous gastrostomy device at home in children with special feeding needs from neuromotor and/or digestive handicaps, checking his utility. Material and Methods: We present 32 children with gastrostomy tube as feeding method, management in ambulatory way, 18 males and 14 females, with ages between 0.4 to 5.5 years (mean 3.4 +/- 1.2 years). They was sending to diagnosis and treatment of their feeding disorders. Before to choice the gastrostomy as a feeding method, it was done diagnostic studies: a. Fluoroscopic study of deglution, esophagic transit and gastric empting. b. 24 hours intraoesophageal pH recording and c. esophagogastroduodenoscopy. When it was decided the use of gastrostomy, it was put in place a per-cutaneous endoscopy tube. As soon as possible, the children were allowed in an ambulatory protocol to manage the feeding system at home, based in the active participation of the families of the children. Results: 23 children (72%) had neurological handicaps with oro-pharingeal dysfunctions to deglution. The other 9 (28%) has anatomic alterations that prevent her normal oral feeding. In 29 children was performed a per-cutaneous endoscopic gastrostomy (91%) and in 3 (9%) it was necessary to do anti-refl ux surgery (Nissen fundoplicature) plus surgical gastrostomy due to the severity of the gastroesophageal refl ux detected. In 6 children occur early complications of gastrostomy: subcutaneous emphysema one child (3 %), intense reactivation of gastro-oesophageal refl ux (3 %) and 4 child with cutaneous injuries peri- ostomy ( 12% ) that resolves with topic measures. All the patients except 2 (6%) show an adequate evolution with the home feeding program.
