Resumen en inglés | Objective: To review the usefulness of helical computed tomography for evaluating margins in preoperative patients with basal cell carcinoma infiltrative referred to conventional surgery. Patients and method: We designed a prospective and descriptive study that included 20 cases of infiltrative basal cell carcinoma, which underwent to simple and contrast helical CT scan of the affected area with the same three-dimensional reconstructions (multi-slice CT), to determine whether the size and scope of the tumor corresponded to those determined by clinical review or whether they were greater. All patients whose margins were similar to the clinical review underwent conventional surgery with a safety margin of between 4 and 5 mm, followed by histological review for the study of the surgical margins. Results: We analyzed 20 patients, 9 men and 11 women with an average age of 66.25 years. The most frequent topography was the nose (40%); the evolution time was 6 months to 10 years (average 3.78 years). In four cases (20%) radiological tumor margins were higher than those seen clinically, so we decided not to treat. Two patients did not complete the study. The 14 cases treated by conventional surgery were reported with histological margins free of tumor. Conclusions: Most skin diseases can be diagnosed by inspection; preclinical diagnostic tools such as computed tomography multi-slice improves the ability of clinicians to diagnose and choose the most appropriate treatment of non-melanoma skin cancer. In basal cell carcinoma infiltrative the image study is accurate and offers to the patient and physician the opportunity to predict the type and extent of surgery required |
