Resumen en español | Se presenta el caso de un paciente masculino de 34 años, con antecedente de artritis de rodillas tratada previamente con corticosteroides, con buena respuesta. Acudió por edema generalizado, coluria y orina espumosa, malestar general y náusea. Por medio de laboratorios, se encontró trombocitopenia, disminución de la función renal, proteinuria en rangos nefróticos y un sedimento urinario con presencia de cilindros granulosos y eritrocitos. En el estudio etiológico del síndrome nefrótico se encontró hipocomplementemia, anticuerpos antinucleares positivos y anticuerpos anti-ADNdc positivos. Se realizó biopsia renal, donde se halló histopatología compatible con nefropatía lúpica difusa (clase IV-G A8 C2, de acuerdo con la clasificación de ISN/RPS 2003), por lo que se inició un tratamiento de inducción con bolos de metilprednisolona (3) y, posteriormente, ciclofosfamida IV (1 g). Luego se agregó fiebre, cefalea, anemia y trombocitopenia, hiperbilirrubinemia indirecta, disminución de haptoglobinas y un frotis de sangre periférica con esquistocitos. Se llegó al diagnóstico de nefropatía lúpica con púrpura trombocitopénica trombótica. El lupus y la púrpura trombocitopénica trombótica se asocian en sólo 2% de los individuos con lupus y los síntomas pueden ser difíciles de distinguir entre las dos patologías. Se ha descrito la relación entre estos dos padecimientos en forma infrecuente, por lo que el curso natural, el pronóstico y los factores de riesgo para la asociación no se conocen exactamente. La sospecha diagnóstica y la prontitud del tratamiento son fundamentales para el desenlace del sujeto |
Resumen en inglés | We present the case report of a 34-year-old male with a history of knee arthritis previously treated with corticosteroids, with good response. He complained about generalized edema, dark and foamy urine, malaise and nausea. His lab test results showed thrombocytopenia, reduced renal function, nephrotic range proteinuria and active urine sediment. In the etiologic work-up for nephrotic syndrome, we found hypocomplementemia, positive anti-nuclear antibodies and positive anti-DNAds antibodies. A renal biopsy was performed, in which we found a histopathology compatible with diffuse lupus nephritis (class IV-G A8 C2, as classified by the ISN/RPS) so he was treated with an induction regimen with methylprednisolone pulses (3) and IV cyclophosphamide (1 g). After the treatment, he presented with fever, headache, higher grade anemia and thrombocytopenia, indirect hyperbilirubinemia, low haptoglobins, and a blood smear with schistocytes. The definitive diagnosis was lupus nephritis with TTP. Lupus and TTP present together only in 2% of all lupus patients, and both pathologies have similar symptoms, so the diagnosis can be easily confused. The relationship between these two entities has been previously described, but it is very infrequent, so the natural history, prognosis, and risk factors for the relationship are not known exactly. The suspicion and promptness of the diagnosis are very important for the positive outcome in these patients |
