Resumen en español | Objetivo: analizar el tiempo transoperatorio y de recuperación posquirúrgica en niños con fractura supracondílea humeral intervenidos por reducción abierta o cerrada con fijación. Métodos: estudio transversal, retrospectivo, comparativo, controlado, no aleatorizado de casos controlados, de 108 pacientes con fracturas supracondíleas humerales tipo II y III de Holmberg. Resultados: se estudiaron 108 casos, 63 masculinos (58.3 %) y 45 femeninos (41.7 %), edad promedio de 8.6 ± 2.7 años; la extremidad derecha fue la más afectada (50.9 %) y el tipo de fractura más frecuente fue el III de la clasificación de Holmberg (77.8 %); el tiempo quirúrgico fue de 60.3 y 30.8 minutos para la reducción abierta y la cerrada; por t de Student se encontró una diferencia estadísticamente significativa (p < 0.05). El grado de flexión fue de 13.6° en el grupo I a las cuatro semanas y de 95° a las ocho; grupo II, 38.7° a las cuatro semanas y 118° a las ocho, con diferencia estadísticamente significativa entre las semanas cuatro y ocho (t de Student p < 0.01). Conclusiones: el tiempo quirúrgico fue menor y la movilización en grados de flexión mejor en los pacientes con reducción cerrada |
Resumen en inglés | Background: humeral fracture (HF) in children requires a specialized management to achieve an adequate reduction and fixation, as well as a prompt attention to prevent complications. Our objective was to show advantages and disadvantages of HF treatment in relation to operating time and post-surgical recovery. Methods: Cross study, retrospective, comparative based on the reviewing of 108 patients’ files with HF type II and III of Holmberg. There were two groups, one underwent an open reduction and internal fixation and the other one a close reduction and pegging. Results: There were 63 male (58.3 %) and 45 Female (41.7 %) with an average age of 8.6 years (range: 4 to 14 years), being the right extremity the most affected in 50.9 %, the most frequent fracture was the type III in 77.8 % (84 cases), the average operating time for an open reduction was 60.3 minutes and for a close reduction 30.8 minutes. Student t showed a statistical difference (p < 0.01), the recovery time was 4 weeks with a flexion of 13.6° for group I. Group II had a 4 week time recovery with an inflection of 38.7°. Conclusions: surgical time was shorter in the closed reduction; flexion at four weeks had a better clinical recovery |
