| Spanish abstract | Diferentes metáforas aplican al anciano, que son en parte adecuadas pero incompletas. No describen apropiadamente su situación personal frente a su decadencia, su vida que se acaba, su proyecto pendiente. En el cirujano esta situación es especial, pues en el envejecer le va no sólo su ejercicio profesional sino las consecuencias sobre sus pacientes. Se abordan varios dilemas; el primero como La espada de Damocles, una metáfora que se ajusta bien a su problema. Vista como dilema, debe decidir si permanece ante la mesa de operaciones, con la espada sobre su cabeza, o se retira, y ambas opciones tienen consecuencias serias para su futuro. El segundo como un péndulo, la decisión entre un altruismo exacerbado ante un panorama económico y social cada vez más desfavorable y el egoísmo del cuidado de sí frente a las exigencias del otro. Uno es sincrónico, el otro diacrónico. El cirujano puede querer cuidarse para llegar a viejo, bajo el riesgo de no cumplir y no tener ya capacidad de consumar lo que aspiraba, o puede prodigarse enteramente al ejercicio de su arte, bajo el riesgo de su morbimortalidad. El dilema es triple; entre los otros y él, entre el ahora y el mañana, entre la contemplación y la acción |
| English abstract | Different metaphors apply to the elderly, which are in part adequate but incomplete. They do not properly describe their personal situation in the face of their decline, their life that is over, their project pending. In the surgeon this situation is special, because in aging not only his professional practice, but the consequences on his patients are at stake. Several dilemmas are addressed; the fi rst, as The Sword of Damocles, a metaphor that fi ts his problem well. Seen as a dilemma, he must decide whether to stand before the operating table with the sword upon his head, or retire, and both options have serious consequences for his future. The second, as a pendulum, the decision between an altruism exacerbated by an increasingly unfavorable economic and social panorama, and the selfi shness of self-care against the demands of the other. One is synchronous, the other diachronic. The surgeon may want to take care of himself to become old, under the risk of not fulfi lling and no longer being able to consummate what he aspired for, or he may be entirely devoted to the exercise of his art, at the risk of his morbidity and mortality. The dilemma is threefold; between the others and his, between now and tomorrow, between contemplation and action |
