Resumen en español Se analiza la alteración de la proteína STAT5b (Signal transducers and activatiors of transcription) que produce activación intracelular y media la acción de hormona de crecimiento (GH) y citoquinas, como Interleucina 2 que es responsable de proliferación y diferenciación de células T. Esta alteración STAT5b produce déficit inmunológico y del crecimiento. Se presenta el caso de una niña de 12 años natural de Córdoba, Argentina que presenta desde el nacimiento déficit de crecimiento con valores normales de GH y bajos de factor de crecimiento insulinoide-1 (IGF-1). Prueba de generación de IGF-1 negativas. Padeció varicela severa que comprometió su vida (a los 4 años). Presenta bronquitis a repetición. Presenta insuficiencia respiratoria crónica (IRC) diagnosticada por clínica y laboratorio. Fue hospitalizada por corpulmonar. Se realiza el diagnostico presuntivo de Inmunodeficiencia Primaria con Déficit del Crecimiento (IDPDC). Se realiza estudio genetico (secuenciación de ADN de la proteína STAT5b). La etiología de IRC fue neumonitis intersticial linfoidea. La paciente fallece por distres respiratorio. Al examen físico: 111cm, 13Kg [ambos -4DS], tos, disnea, roncus, sibilancias, signos de hipoxia crónica y dermatitis descamativa periorbitaria. Recibio tratamiento de mantenimiento durante el tiempo de enfermedad. El presente caso clínico evidencia la relación entre el sistema endocrino e inmunológico, la alteración de STAT5b modifica la respuesta de estos sistemas
Resumen en inglés The STAT5b (signal transducers and activators of transcription) protein alteration that produces intracellular activation is here analyzed. STAT5b mediates the action of growth hormone (GH) and cytokines like interleukin 2 (IL-2) which is responsible for the proliferation and differentiation of T-cells. This anomaly produces both immunologic and growth deficit. Patient A 12-year-old girl, from Córdoba, Argentina. She presents growth deficit from birth with normal GH values and low values of growth factor similar to insulin-1 (IGF-1). IGF-1 generation test was negative. She suffered from severe varicella that endangered her life at the age of 4. She had repeated episodes of bronchitis. She has chronic respiratory failure diagnosed both clinically and by laboratory studies. The patient was admitted to hospital because of cor-pulmonale. A presumptive diagnosis of primary immunodeficiency with growth deficit. The Garraham Hospital of Buenos Aires, Argentina was consulted for making a genetic study (DNA sequencing of the STAT5b protein) The etiology of chronic respiratory failure is lymphoid interstitial pneumonitis. The girl died of respiratory distress. Physical examination: 111cm (-4SD), 13 kg (-4SD), cough, dyspnea, wheezing, signs of chronic hypoxia and periorbitary descamative dermatitis. Studies: Thyrotrophin, free tyrosine, plasmatic cortisol, GH, IGF-1, test of IGF-1 generation, immunoglobulin, Antistreptolysin- O (ASO), complement (C3, CH50), Nitroblue Tetrazolium Test (NBT Test), isohemaglutinins, total lymphocytes, lymphocytes T, TCD4+, TCD8+, thorax x-ray film, spirometry, gasometry, electrocardiogram, doppler echocardiography, lung biopsy, karyotype, DNA sequencing for STAT5b. Maintenance of the clinical picture. The relation between the endocrine and immunologic system is evident. The alteration of STAT5b modifies the response of these systems
