Resumen en español Objetivo: comparar costos y resultados en salud de duloxetina 60 mg QD (DUL60 mg), pregabalina 150 mg BID (PGB300 mg) y pregabalina 225 mg BID (PGB450 mg) para tratamiento de fibromialgia (FM) desde las perspectivas del sistema público y privado de salud en México. Material y métodos: se efectuó una búsqueda bibliográfica de ensayos clínicos multicéntricos, aleatorizados, doble-ciego y controlados por placebo que evaluaron estrategias competitivas en pacientes adultos con FM. Mediante comparación indirecta ajustada, se determinaron las probabilidades de alcanzar respuesta satisfactoria (definida como alivio del dolor 30% al final del tratamiento con respecto a la medición basal) y de abandono prematuro. El horizonte temporal del estudio fue 3 meses. Se analizaron costos de adquisición de medicamentos y consultas médicas según tipo de respuesta, expresándolos en pesos mexicanos de 2012. Las fuentes de información incluyen listados oficiales y literatura publicada. El análisis de sensibilidad fue determinístico y probabilístico. Resultados: bajo ambas perspectivas, DUL60 mg resultó dominante (menos costoso y más efectivo que PGB300 mg y PGB450 mg). Los ahorros promedio por paciente al emplear DUL60 mg fueron 93.10 y $ 1,825.76 (perspectiva pública) y 383.61 y $ 4,936.63 (perspectiva privada) con respecto al uso de PGB300 mg y PGB450 mg, respectivamente. Por cada mil pacientes tratados con DUL60 mg en lugar de con PGB300 mg (PGB450 mg) habría 45 (6) pacientes adicionales con respuesta satisfactoria (527 vs 482 vs 521). DUL60mg fue costo-ahorrador en alrededor de 70% de las simulaciones generadas. Conclusiones: los resultados sugieren que DUL60 mg ofrece ahorro y mejorias en salud en comparación al uso de PGB300 mg para el tratamiento de FM en México
Resumen en inglés Objective: to compare the costs and health outcomes of duloxetine 60 mg QD (DUL60 mg), pregabalin 150mg BID (PGB300 mg) and pregabalin 225 mg BID (PGB450 mg) in the treatment of fibromyalgia (FM) from the perspective of public and private healthcare systems in Mexico. Methods: a systematic review of multicenter, double-blind, randomised and placebo-controlled clinical trials evaluating the competitive treatment strategies in adult patients with FM was performed. Probabilities of achieving satisfactory response (defined as an improvement 30% in pain relief from baseline) and early withdrawal were derived by conducting an indirect comparison. Time horizon was set at 3 months. Only direct medical costs comprising acquisition of study drugs and medical visits by category of response were analyzed and they are expressed in 2012 mexican pesos. Sources of information include official listings and published literature. Deterministic and probabilistic sensitivity analyses were undertaken. Results: DUL60mg demonstrated to be the dominant strategy (less cost and more effective) compared to PGB300 mg and PGB450 mg under both healthcare perspectives. The mean savings per patient with DUL60 mg were $93.10 and $1,825.76 (public perspective) and $383.61 and $4,936.63 (private perspective) compared to PGB300 mg and PGB450 mg, respectively. For every 1000/patients treated with DUL60 mg instead of PGB300 mg and PGB450 mg, 45 and 6 additional patients would achieve satisfactory response (527 vs. 482 vs. 521, respectively). DUL60mg was cost- saving in nearly 70% of the simulations. Conclusion: these results suggest DUL60 mg offers both savings in total costs and improvements in health outcomes compared to PGB300 mg and PGB450 mg for the treatment of FM in Mexico
