Resumen en español Antecedentes: La hemorragia de tubo digestivo es la pérdida sanguínea en cualquier segmento del tubo digestivo; puede ser alta o baja. No contamos con clasificaciones de choque hipovolémico no traumático que ayuden a determinar la gravedad o necesidad de transfusión en estos pacientes. El déficit de base es un marcador que podría indicar la gravedad y necesidad de transfusión. Objetivo: Determinar si el déficit de base es predictor de gravedad e indicador de transfusión en pacientes con hemorragia de tubo digestivo y choque. Material y métodos: Estudio prospectivo, bicéntrico, descriptivo, observacional; incluimos pacientes que ingresaron con diagnóstico de hemorragia del tubo digestivo a la sala de urgencias del Centro Médico ABC del 1 de enero de 2016 al 1 de enero de 2017. Resultados: Los pacientes con mayores niveles de déficit de base (clases III y IV) tuvieron menor tensión arterial media, mayores niveles de lactato, nitrógeno ureico en sangre y calificación de APACHE II. La mortalidad fue mayor en las clases III y IV, con tendencia a la significancia estadística. El uso de vasopresores, intubación y necesidad de ingreso a la Unidad de Cuidados Intensivos fue mayor en pacientes con clase III y IV. La más alta cantidad promedio de paquetes por paciente se observó en la clase III, con mediana de cinco paquetes. Conclusiones: El déficit de base amplio en pacientes con hemorragia de tubo digestivo predice gravedad y necesidad de transfusión temprana y con mayor número de paquetes globulares, así como estancia prolongada en pacientes con choque secundario a hemorragia de tubo digestivo
Resumen en inglés Background: Gastrointestinal bleeding is blood loss in any segment of the digestive tract; it can be divided into upper and lower. It frequently occurs with hypovolemic shock. We do not have non-traumatic hypovolemic shock classifications that help determine the severity or need for transfusion in these patients. Base deficit is a marker that could determine the severity and need for blood transfusion. The aim of this study was to determine if the base deficit is a predictor of severity and indicator of transfusion in patients with gastrointestinal bleeding and shock. Material and methods: Prospective, bicentric, descriptive, observational study; we included patients admitted with a diagnosis of gastrointestinal bleeding to the emergency room of any of the campuses of the ABC Medical Center from January 1, 2016 to January 1, 2017. Results: Patients with higher levels of base deficit (classes III and IV) had lower mean arterial pressure, higher lactate levels, blood urea, II score. Mortality was higher in classes III and IV, with a tendency to statistical significance. The use of vasopressors, intubation and need for admission to the intensive care unit was higher in patients with classes III and IV. The highest average number of units per patient was observed in class III, with a median of five units. Conclusions: A higher base deficit in patients with gastrointestinal bleeding can predict severity, the need for early blood transfusion and a greater number of red blood cell units, as well as a prolonged stay, need for vasopressors and admission to the intensive care unit in patients with shock secondary to gastrointestinal bleeding
