| Spanish abstract | OBJETIVO: revisar la bibliografía más reciente y determinar si las principales alteraciones en la gestación y desarrollo fetal, generadas por el hipotiroidismo materno, pueden evitarse con el diagnóstico oportuno, si una prueba temprana de tamizaje es útil y si el tratamiento hormonal sustitutivo es benéico. CONCLUSIONES: está suicientemente demostrada la trascendencia de la hormona tiroidea durante todo el embarazo; los eventos que se generan desde el inicio son decisivos para deinir complicaciones y alteraciones en el feto y la gestación. Hoy día, la sospecha clínica y las pruebas disponibles para establecer el diagnóstico son insuicientes y poco certeras. Las nuevas tecnologías requieren ser menos complicadas y accesibles y hacen falta más estudios para determinar si la intervención terapéutica de reemplazo hormonal (levotiroxina) hace una diferencia y previene complicaciones en el desarrollo fetal, neonatal y la salud del embarazo |
| English abstract | OBJECTIVE: To review the most recent literature and determine if major changes in pregnancy and fetal development, generated by maternal hypothyroidism can be avoided with early diagnosis, if an early screening test is useful and if hormone replacement therapy is beneicial. CONCLUSIONS: It is suficiently demonstrated the importance of thyroid hormone throughout pregnancy; events that are generated from the beginning are crucial to deine complications and abnormalities in the fetus and pregnancy. Today, clinical suspicion and tests available for diagnosis are inadequate and not very accurate. New technologies need to be accessible and less complicated and more studies are needed to determine whether hormone replacement therapeutic intervention (levothyroxine) makes a difference and prevent complications in fetal, neonatal and pregnancy health development |
