Spanish abstract OBJETIVO: describir la técnica Posadas, que mediante la ligadura selectiva de las arterias uterinas, con abordaje posterior y ligadura de la rama terminal de la arteria ovárica (tubaria interna), es una opción de tratamiento conservador de la hemorragia obstétrica posparto. CONCLUSIÓN: la hemorragia posparto es la primera causa de muerte materna. Representa una urgencia obstétrica que debe tratarse por personal adiestrado. El procedimiento adecuado es fundamental para evitar el sangrado y sus complicaciones. Cuando los métodos comunes para disminuir el sangrado obstétrico fracasan, es necesaria la intervención quirúrgica. Se han propuesto diversos abordajes quirúrgicos, que sean reproducibles, técnicamente sencillos y costo-efectivos. La ligadura bilateral de las arterias uterinas la han descrito diferentes autores como una alternativa efectiva y segura para tratar pacientes con hemorragia obstétrica posparto. La técnica descrita combina lo mejor de otros procedimientos y se caracteriza por su efectividad, rapidez, seguridad y reproducibilidad
English abstract OBJECTIVE: To describe a new surgical method, the “Posadas” Technique which consists in the posterior ligation of the uterine artery and the tubo-ovarian artery. This technique certainly upgrades the management of PPH, reduces the rate of hysterectomy and helps preserve subsequent fertility. CONCLUSION: Postpartum hemorrhage (PPH) is the major cause of maternal death worldwide. It is an obstetric emergency that requires a skillful obstetrician to treat it. Initial management is essential to control bleeding and to avoid complications. If the usual methods fail to stop the hemorrhage, surgery is inevitable. Many surgical techniques have been employed to stop intractable postpartum hemorrhaging. Technically simple hemostatic procedures have proved to be fast, safe, economical and highly efficient. Bilateral ligation of the ascending branches of the uterine artery has been reported by multiple authors as a simple, safe, effective and life-saving alternative in the management of uncontrollable obstetric hemorrhage. The Posadas technique combines the best of the other methods, its main quality is simple, rapid, practical and reproducible method
